What You’ll Do

• Conduct patient admission assessments and develop care plans.

• Perform supervisory visits to monitor patient care and progress.

• Educate patients and families on medications, conditions, and care plans.

• Document visits accurately and communicate changes in patient condition.

• Collaborate with physicians and the healthcare team to ensure quality care.

What We’re Looking For:

• Licensed Registered Nurse (RN) in Georgia

• Experience in home health, long-term care, or geriatrics is a plus

• Strong verbal and written communication skills

• Ability to lift up to 40 lbs using proper body mechanics

• Compassionate, reliable, and self-motivated—with a cool head in tough situations
