Are you an RN who thrives on autonomy and building meaningful, one-on-one relationships with your patients? We are seeking a dedicated Full-Time RN Case Manager to join our team in Indianapolis. In this role, you’ll trade the hectic hospital hallways for the comfort of your patients' homes, providing high-quality care where it matters most.

The Role & Schedule

As a Case Manager, you are the heartbeat of our patient care. You will oversee a dedicated caseload, ensuring comprehensive care coordination and clinical excellence.

• Status: Full-Time (Minimum 40 hours per week).

• Territory: Patients are primarily located within the Indianapolis area. While most travel is within a 30-minute radius, flexibility for up to a 45-minute drive may be required occasionally.

Compensation & Benefits

• Annual Salary: $75,000 – $80,000.

• Mileage Reimbursement: $0.50 per mile.

What You’ll Do

• Perform initial and ongoing clinical assessments.

• Develop and implement personalized plans of care in collaboration with physicians.

• Educate patients and families on disease management and self-care techniques.

• Coordinate with a multidisciplinary team to ensure seamless transitions and holistic support.

• On-Call: Rotation is approximately every 4–6 weeks (1 full week).

Qualifications

• Current, unencumbered Registered Nurse (RN) license in the state of Indiana.

• Minimum of 1 year of clinical nursing experience (Home Health or Case Management experience preferred).

• Valid Driver’s License, reliable transportation, and active auto insurance.

• Strong organizational skills and the ability to work independently.
