Job Description
We are seeking a skilled and compassionate Registered Nurse (RN) who is fluent in Bosnian and English to support our Adult Family Living (AFL) program in the Hartford, CT area.
In this role, you will be a vital clinical resource for our Bosnian-speaking participants and their caregivers. As an RN, you will provide the professional oversight necessary to ensure participants are thriving in their home environments. Your ability to communicate complex medical information in a culturally and linguistically appropriate manner is essential to the success of this program.
Responsibilities
Conduct clinical wellness visits for participants enrolled in the AFL program.
Communicate fluently in Bosnian and English to perform assessments and provide health education to caregivers.
Monitor participant health status, evaluate medication management, and identify any changes in condition.
Complete all RN clinical documentation in compliance with Connecticut state regulations and agency policies.
Act as the primary clinical liaison between the participant’s family and the healthcare team.
Qualifications
License: Must hold a current, valid Registered Nurse (RN) license in the State of Connecticut.
Language: Must be fully bilingual in Bosnian and English (spoken and written).
Clinical Skills: Strong assessment and critical thinking skills are required.
Experience: Previous experience in home health, community nursing, or AFL/Caregiver programs is highly preferred.
Requirements: Valid driver’s license and a reliable vehicle for travel to visits within the greater Hartford area.
