Job Summary

We’re looking for skilled and caring Registered Nurses to join our Connecticut home care team. This role offers the autonomy of field work, the support of an experienced clinical team, and the chance to build meaningful patient relationships.

Both Full-Time and Per Diem roles are available.

Responsibilities

• Provide skilled nursing visits and complete comprehensive assessments

• Create, implement, and update individualized care plans

• Deliver patient/caregiver teaching, medication management, and wound care

• Communicate regularly with physicians, therapists, and the care team

• Document all care accurately and on time

• Maintain professional boundaries and promote patient safety at all times

Qualifications

• Active Connecticut RN license (required)

• Preferred: Home health experience, including OASIS

• Accepted: Hospital, med-surg, telemetry, LTC, SNF, or rehab

• Strong clinical assessment skills

• Valid driver’s license, reliable vehicle, and auto insurance

• Excellent communication and documentation skills

• Monday–Friday

• Rotating weekend/holiday coverage

• Productivity bonus once visit thresholds are met

• Premium salary $93,000.00 to $104,000.00
