Join our well-established Home Health team as an LPN!

Available: Full-time/ Part-time/ Per-Diem

Location: New Haven County, CT and surrounding areas

This role offers the autonomy of field work, the support of an experienced clinical staff, and the opportunity to build meaningful one-on-one patient relationships.

Schedule:

• Monday–Friday daytime hours for strong work–life balance

• Extra pay for rotating weekend/holiday coverage

• Flexible scheduling options available

Qualifications:

• Active Connecticut LPN license (required)

• Minimum 6 months of clinical experience (hospital, LTC, SNF, rehab, med-surg, or similar)

• Valid driver’s license, reliable vehicle, and can travel to visit clients

• Strong assessment, communication, and documentation skills

• Spanish speaking is a plus!

• Home health experience is a plus!

Responsibilities:

• Provide skilled nursing visits in the patient’s home

• Document vitals as well as care during visits, and report changes in condition

• Implement and update individualized care plans under RN supervision

• Deliver patient and caregiver teaching

• Assist with medication management and wound care

• Communicate effectively with physicians, RNs, therapists, and the care team

• Maintain professional boundaries and promote patient safety at all times
